Volunteers who serve with our children at Family Life Church are required to have a volunteer application and the State mandatory background checks on file. You can fill out the full packet and return to Lisa Babcock in the church office. Click here for the PDF.

DOCUMENTS NEEDED ON FILE:

• Application and Release of Liability.
  • We are required to obtain a State Background check of our own even if you provide a copy.
  • Release of Liability to protect you and Family Life Church (aka, First Assembly of God)

• FBI Fingerprint report or Affidavit Disclosure:
  • If you have lived in Pennsylvania for 10 years or more you can fill out the Affidavit Disclosure
  • or an Ohio resident or lived for less than 10 years in Pennsylvania, you need to provide a fingerprint report,  use service code:1KG6ZJ  (Pennsylvania DHS-Volunteer)  there is a cost to obtain.

• Child Abuse History Report: 
  • If you have a report from your employment you can provide a copy
  • or fill out the application along with the Consent/Release of info form
  • or go to the State’s Compass page to create an account and provide a copy.
