STATE-REQUIRED CLEARANCES:  Pennsylvania State Child Protective Services law requires that all volunteers who come into contact with or are responsible for children submit their clearances every 60 months.  Download our Fact Sheet on how to obtain your own PA State Required Clearances to volunteer at Family Life Church.

You may choose to complete the documents and return them to Lisa Babcock in the church office to be processed. Provide the following:

• Application for 18+ year olds
• Application/Release 13-17 years of age
• FBI Disclosure Statement (PA Resident 10+)
• FBI Fingerprint Instructions (Non-PA Resident or under 10 years in PA)
• PA Child Abuse History Report Application and Release
