Volunteers who serve with our children at Family Life Church are required to have a volunteer application and the State mandatory background checks on file.  Click here for the PDF.

DOCUMENTS NEEDED ON FILE:

• Application and Release of Liability.
  • We are required to obtain a State Background check of our own even if you provide a copy.
  • Release of Liability to protect you and Family Life Church (aka, First Assembly of God)

• FBI Fingerprint report or Affidavit Disclosure:
  • If you have lived in Pennsylvania for 10 years or more you can fill out the Affidavit Disclosure, or
  • An Ohio resident or lived for less than 10 years in Pennsylvania, you need to provide a fingerprint report,  use service code:1KG6ZJ  (Pennsylvania DHS-Volunteer)  there is a cost to obtain.

• Child Abuse History Report: 
  • If you have a report from your employment you can provide a copy, or
  • Fill out the application along with the Consent/Release of info form, or
  • Go to the State’s Compass page to create an account and provide a copy.
